no longer make legal decisions for their kids as soon as a child turns 18. What’s more is that
they also no longer have any special “rights” to receive private information, such as health care information, without the child’s permission

Healthcare Power of Attorney:
The healthcare power of attorney form allows you to designate someone to make future healthcare decisions on your behalf if you become too ill or unable to make those decisions yourself.
HIPAA Waiver:
The HIPAA waiver allows a designated individual or third party to receive and request information regarding your health.

A: No, either just the parent(s)/guardian(s) can attend or just the student can attend.
Q: Are there any additional documents we need to bring?
A: Photo ID is required if you plan to register your student with the Arizona Health Care Directives Registry.
Q: What if my student is above the age of 18?
A: Any student aged 17 1/2 or older can attend and complete these documents.
